Becoming a New Patient

What to expect from your first visit ?

An initial appointment consists of a comprehensive interview covering your personal history and that of your concern as well as physical examination and an individualized treatment plan to address your concern. This initial visit takes approximately 90 minutes.

Initial online intake forms

After calling our office, you will be requested to set up a Patient Ally account which provides you with the intake documents and opportunity to have communication with your doctor via a HIPPA compliant messaging system.

We will send you a link to your email from which you follow the instructions to set up a username and password.

Just a reminder, you will need to access Patient Ally on a computer, it unfortunately does not work on a tablet or phone.

Click on the ‘documents icon’ and on the left hand side are the words ‘intake documents’ in blue lettering, click on that and your seven documents will be visible.

 If you have any trouble with this online process, you can come into the office 30 minutes prior to your appointment to fill out on our tablet.

Cancellation/ Reschedule Courtesy

If you have questions before your appointment please don’t hesitate to call. If for any reason you need to cancel or reschedule your appointment we ask that you give 48 hours notice.

Insurance

We are preferred providers on the First Choice Health Network, Premera Blue Cross, Regence Blue Shield, Kaiser, Aetna, United Healthcare and Uniform Health Plans.

If you have other insurance we would be happy to provide you with a form to submit for partial reimbursement. Reimbursement varies so please call your insurance carrier to determine your coverage for “out-of-network” providers.

Also, please contact your insurance prior to your visit to ensure your co-pay amount and coverage with our ND.
